Netscape 6.0
Netscape 6.0 has a bug that automatically selects the first radio button in every set of radio buttons when you print a page that contains a form. This bug seems to be unique to Netscape 6.0, and for completing this survey, it is recommended that you use an alternate browser.

Security Certificate
The Internet site you are trying to view uses a security certificate that was signed by an unknown authority. This page cannot be viewed. - The browser you are using probably does not have Thawte (Verisign) certificate installed. Install the Thawte root to your browser at this page:

How to Login
To open the survey, you must first log in using your Login and Survey ID. Use your mouse pointer and click in the box labeled . Type in your Login and verify that it is typed correctly. Then use your mouse pointer to click in the box labeled . Type in your Survey ID. To log in and begin filling out the survey, press the login button below those boxes. If you have forgotten either your Login or your Survey ID, click here.

Using Radio Buttons
To select a radio button, move your mouse pointer over the radio button you wish to select and click once. If you want to change your answer just click another radio button under the same question and your previous mark will be deleted.

Using Checkboxes
Selecting a check box is almost exactly like selecting a radio button. Move your mouse cursor over the check box you wish to select and click once. Check boxes work a bit differently when it comes to de-selecting it as compared to radio buttons. With check boxes one must actually re-click the selection again for it to go away because more than one check box can be selected under a single question.

Using Textboxes
To put your answer into a text box move your mouse cursor over any part of the text box and click once. Then just type in your answer using the keyboard. To delete an answer double-click in the box (the text should now be highlighted) and then press either the "Backspace" or "Delete" key. When you have the correct answer in the text box, proceed to the next question or submit button.

Script Errors
"My computer said it had a script error."
"I got a JavaScript error message saying 'screen is not defined.'"
"JavaScript Error:..."

If you received any of these or similar errors, the browser you are using probably does not support JavaScript or JavaScript has been disabled. We recommend using these Web browsers:

Netscape 2.x or greater
Microsoft Internet Explorer 3.x or greater
